EVOLVES Vascular Lab is an on-site ultrasound facility which we formed in September 2006 to provide not only vascular surveillance of our non-cardiac surgical patients, but to also treat varicose veins.   We are proud  to state  that  our  lab  is staffed with a full-time Registered Vascular Technician (RVT) certified by the ARDMS.  Having the on-site services is an added convenience for our patients, providing that extra assurance that the follow-up vascular care and treatment is by their own CVT surgeon.

If you are interested in having your ultrasound services performed in our facility, just click and print our Referral Form and provide it to your physician to be referred to us.  Once appointment has been scheduled, just click, print and complete our New/Returning Patient Form and bring it with you to your appointment. 

Since late 2007, we have been providing treatment and follow-up for patients with varicose veins.  We have had great success from using the VNUS Closurefast RF Ablation procedure.  Venous insufficiency symptoms vary with each patient, and it may take several appointments and attempts to completely resolve your symptoms, but overall we have had very successful and positive feedback from our patients.  We look forward to providing other venous services in the future.   Many major insurance companies now realize that varicose veins and venous reflux disease are more than just "cosmetic".  Provided that each patient has met the medically necessary requirements for the treatment of their varicose veins, there should not be any denials by their insurance companies.  Here are the necessary forms that need to be completed to be referred to our practice for varicose veins:  Referral Form to take to your physician to schedule the appointment with our office, New/Returning Patient Form and Venous Health History Form.

Please be aware that if an appointment is scheduled for you, and you do not contact our office within 24-hours to reschedule or cancel your appointment, you will be charged a missed appointment fee which will be billed directly to you, not to your insurance company.  Another patient needing our services will have the opportunity to fill your appointment cancellation.
